DATA CONTROLLER APPLICATION FORM

1. GENERAL

This Application Form has been prepared by Armine Giyim Sanayi ve Ticaret Anonim Şirketi (“Armine” or the “Company”) as a data controller, to promptly, effectively, and comprehensively evaluate and resolve applications made by data subjects under Articles 11 and 13 of the Personal Data Protection Law No. 6698 (“KVKK”).

2. APPLICATION METHODS

As data subjects, you may submit your requests related to the implementation of the KVKK to Armine, as the data controller, by filling out this application form and using one of the following methods:

☐ Submitting a signed copy of this form in person to our address: Topçular Mahallesi, Sahabeler Caddesi, No:3/5, Sıla İş Merkezi, Eyüp, Istanbul.

☐ Sending a signed copy of this form to our registered electronic mail (KEP) address: [email protected], using your registered electronic mail or mobile signature.

☐ Sending a signed copy of this form via email to [email protected] using the email address previously registered in the Company's system.

4. DATA SUBJECT REQUESTS
